In addition, by submitting my application, I understand and agree to the following:

Chatham accepts children from all races, national origins and religious backgrounds.
Enrollment is based on a "first come, first serve basis" only.
Children will NOT be enrolled unless an immunization record is presented and immunizations are up-to-date.
Children need to be 3+ months for the infant/toddler program.
Children need to be at least 2 years of age for the preschool program.
I will be notified in writing whether or not my child has been accepted. As required at the time of application, I am submitting the application fee of $25, if my child is accepted, I will submit the yearly non-refundable registration fee of $125.00.
Once accepted via a written notice, I will then be informed regarding the availability of my requested schedule.
